Infectious and Contagious Illness Reporting
Per Communicable Disease Rules 3703-3-01 through 3701-3-31 of the Ohio Administrative Code (OAC), mandated reporting of specific illnesses is required to the local health department(s) with jurisdiction. The Ohio Department of Health posts reportable infectious/contagious disease guidance online: Know Your ABCs: A Quick Guide to Reportable Infectious Diseases in Ohio. In addition, the U.S. Centers for Disease Control and Prevention issues alerts and guidance as it relates to infectious/contagious disease and pandemic-level events at www.cdc.gov.
The following steps should be followed if you suspect or become aware of a member of the College community potentially contracting, exhibiting symptoms, or having been exposed to a carrier of an infectious/contagious disease:
- Please contact Risk.Management@joe-yan.net and, if appropriate, ask the individual to do the same. Provide as much detail about the situation as possible.
- Complete and submit a Cuyahoga Community College Employee Incident Report Form or Student Incident Report Form to Risk.Management@joe-yan.net for any incidents of illness that occur onsite at the College.
- Tri-C Compliance & Risk Management will investigate the claim, request room cleaning(s), restrict or quarantine applicable individuals, and conduct contact tracing, as necessary.
Please note:
- Release/disclosure of information will be limited to appropriate personnel from the College and public health entity(ies) with jurisdiction including, but not limited to the Cuyahoga County Board of Health, the Ohio Department of Health, the Centers for Disease Control and/or local public health entities (i.e., the City of Cleveland).
- In accordance with FERPA, no additional information outside of the specific purposes outlined above may be released/disclosed without additional signed, written authorization from the student and/or the student's legal guardian.
- Any Tri-C employee, student, agent, visitor, vendor or contractor of the College directly involved in food handling/preparation should also self-disclose pertinent symptoms or diagnosed illness in accordance with Ohio Uniform Food Safety Code §3717-1-02.1.
Exclusion and Restriction from College Locations
Anyone exhibiting the symptoms of infectious/contagious illness may be restricted or quarantined from College locations by Tri-C Compliance & Risk Management or under the direction of Public Health entity(ies) with jurisdiction.
